The formal version using Вы. Used with strangers, older people, or in professional settings. It is somewhat formal and signals respect.
The standard positive response. Russians may give more candid answers than "fine" in casual conversation, but this is always an appropriate polite reply.
Polite. With friends: а ты? (a TY).
Literally "how do they call you?".
Literally "it pleases me".
