Apa khabar means what news, the formal how-are-you. Anda is formal you. Used when meeting elders, in business settings, or with people you do not know well. Less common in casual Malaysia.
Literally "news good". The standard answer, always positive. "Sempurna" (perfect) or "Biasa saja" (just ordinary) work too, though biasa saja is considered slightly negative or humble.
