For neighbors older than you, shopkeepers you see daily, anyone you address formally. Unlike the American "how are you", this can invite a real answer, so keep walking slowly.
The standard answer. Bulgarians often add a little realism: "горе-долу" (GO-reh DO-loo), so-so, literally "up-down", is an extremely common and honest reply.
The polite return question. Among friends: "а ти?" (a TEE).
